Individualized Care

At Lakeside Park we believe that many actions are an attempt to communicate. This means we focus on understanding what an individual is trying to express, rather than attempting to control or stop behavior.

Every member of our staff understands the importance of being where the resident is at with unconditional love, respect and understanding. We encourage everyone to develop relationships with each resident and to look for simple and creative solutions that are as unique as each of us.

The individual service plans are developed with the help of all those directly involved in a resident’s care. This helps us to ensure that we are meeting changing needs and allows us to share information with other health care providers to address short and long-term needs.

Service plans include information about the personal likes and dislikes of each resident, helping us design a plan for providing support to accomplish: personal care needs, medications management, social, spiritual, and recreational requests, and health care monitoring.
